The health risk assessment (HRA) visit consists of a face to face interview by a nurse practitioner in the patients’ home. The interview will be approximately 20 – 25 minutes and provide you and your physician with information about your health and how well you are doing. It is an opportunity to for you to share information and assist in the quality and level of care you are receiving from your physician. Why should every patient have a HRA visit each year? · The health risk assessment visit enables an in depth assessment of key diseases and the state of the patients health on an annual basis · Guides medical providers in their evaluation of patients and documentation of care planning · The HRA captures diagnosis codes and key Healthcare Effectiveness Data and Information Set (HEDIS) measures · The HRA accurately and completely captures diagnosis codes to ensure regulatory compliance What happens during the HRA visit? · The Nurse Practitioner will review patient demographics · Record vital signs · Review medications · Interview the patient using BPHP tool for confirming diagnoses · A copy of the information gathered during the HRA visit will be sent to the patients Primary Care Physician, along with any recommendations of missing labs, tests and HEDIS related clinical procedures needed. The Nurse Practitioner will NEVER: · Ask the patient for personal information such as a Social Security number · Ask the patient to change their Primary Care Physician or insurance company · Ask the patient to sign any documents
