Our nurse practitioners help with all aspects of patient care, including health assessments, diagnosis, treatments and consultations. The majority of our business is related to in-home health assessments performed for Medicare Advantage Programs. Generally, our nurse practitioners perform health assessments and educate patients about preventative care and the prescribed treatments, particularly those with chronic conditions. RESPONSIBILITIES • Perform health risk assessments in our patient's home • Document any chronic conditions with appropriate diagnoses codes for HCC categories • Assess and document any situations that should be brought to the attention of patient's Primary Care Physician • Direct Medical Assistant in administering any tests during the patient visit • Write orders for any follow up tests that need to be administered with results sent to patient's Primary Care Physician SALARY/COMPENSATION Nurse Practitioners may be paid on an hourly or salaried basis depending upon their work preference and hours needed in their market area. Pay is commensurate with background and experience. Full time employees (32 hours or more per week) are eligible for the employee benefits program. JOB REQUIREMENTS Nurse Practitioners should be licensed within the state of the market area worked and should be approved and credentialed by Medicare. CAQH and/or current credentialing with our client health plan are desirable but not required.
