• What is the cost to me for this visit?

o There is no charge. Your insurance company provides these wellness visits once per year to assess your health and keep your Primary Care Physician updated on your condition.

• How long will the visit last?

o The visit should only take about 20 minutes of your day.

• What is the purpose of the visit?

o Your insurance company and your Primary Care Physician want to make sure you are receiving the best care possible. This yearly visit helps keep all your information up to date and will let your doctor know if there is anything you are concerned about regarding your health. Your doctor receives a full report after the visit to update their patient files.

• Does this change my plan?

o No. These visits by our Nurse Practitioners do not change your insurance plan or benefits paid under the plan.

• Can you send me a new insurance card?

o No, our Nurse Practitioners are healthcare professionals and do not handle ID cards nor benefit plan related questions. You need to contact your insurance company’s customer service line to request a new card.

• Does the nurse speak Spanish?

o Most of our Nurse Practitioners are bi-lingual. If he or she is not bi-lingualwe will have a medical assistant there to help with translation.

• How do I prepare for my visit?

o Have all of your prescribed and over-the-counter medications ready for the Nurse Practitioner to review.

o Have an area in your house in mind and ready for the Nurse to conduct the Assessment.

• What if I need to cancel my appointment?

o Please contact one of our schedulers to let them know. They will leave you a number to get in touch with them or you can call toll free: 1-888-807-9272.

o Cancelling the day of your appointment leaves our Nurse Practitioner without a time slot filled in their day. We strive to maximize their efforts each day so we see and work with as many patients as possible. It’s very hard for us to get a replacement appointment the day of. So please try to contact us if you know ahead of time that you cannot complete the assessment.
