Best Practice is committed to quality healthcare for your membership. We meet that commitment to you by working with your members' physicians to document the member conditions and assist with care plans. Our Nurse Practitioners typically make annual visits to the member's home and document the presence of chronic illnesses, if any, and update their physician on their condition. After the visit, we submit our findings to you in a format that meets CMS record keeping requirements for HCC categories as well as recording HEDIS related measures for quality of care.
