Our Nurse Practitioners make in-home visits with your patients and then send you the medical record of that visit. We take care of the majority of HCC and HEDIS reporting requirements with an annual visit. Many of our payer clients offer programs that compensate you and your staff for assisting with our effort and adding any information gathered to your patients' medical records.
